What does it mean when the clk LED does not light up?  Also, the POST code starts at 00 and never changes.
While I am at it, is there a user manual for this anywhere?  I can't find one on the web site.

The instructions are on the back of the cardboard the post card came packaged in. 
CLK refers to the PCI Clock.  The CLK LED being out indicates there is no PCI clock present that the card can detect. You may have to set the jumpers on the card depending on the type of motherboard you are using. 

From what you describe you need to set the jumpers for:
Pure POST port without synchronous PCI CLK

Here is an excerpt from the back of the packaging:

JP1 1-2    JP2 3-4
Short      Short       Intelligent mode 1
Short      Open        Intelligent Mode 2
Open       Short       Pure POST port with synchronous PCI CLK
Open       Open        Pure POST port without synchronous PCI CLK

*Intelligent mode 1 - This mode simulates the presence of a PCI device for motherboards which do not send some signals to the PCI bus unless a PCI card is detected.

*Intelligent mode 2 - This is an alternate PCI device emulation mode to intelligent mode 1.

*Pure POST port with synchronous PCI CLK - Performa s pure POST latch for most standard motherboards which have a PCI clock (CLK) present on the PCI bus slot.

*Pure POST port without synchronous PCI CLK - Performa s pure POST latch without the PCI clock. This should work for most motherboard designs.

POST codes are different for each type of motherboard.  Bios Central - http://www.bioscentral.com/ is a good place to start and remember, Google is your friend.
